The Speech Therapist (SLP) provides speech or language therapy services to individuals and families in their homes.

Examples of Duties and Responsibilities:

• Initial and ongoing assessment to determine level of functioning
• Recommendations of mechanisms to enhance the patient’s speech, language, hearing and swallowing ability
• Developing plan of care
• Identifying barriers to desired outcomes
• Evaluating the effectiveness and outcomes of care
