The Social Worker (SW) provides social work services to patients and their families in their homes.

Examples of Duties and Responsibilities:

• Assessment of psychosocial status of patient and social and emotional factors related to the patient’s illness, need for care, and response to treatment
• Goal setting
• Provides short-term individual counseling, community resource planning and crisis intervention to patients and families
• Facilitates referrals of patients and families to appropriate community resources
