The Physical Therapist (PT) provides physical therapy services to individuals and their families in their homes according to the plan of care.  Physical Therapists exercise supervision on Physical Therapy Assistants (PTA).

Examples of Duties and Responsibilities:

• Patient care activities to increase the patient’s function and independence
• Development of plan of care
• Identification of barriers to desired outcomes
• Goal setting
• Evaluation of effectiveness and outcomes of care
