The Personal Care Provider, working under close supervision provides unskilled personal care and household services in compliance with the service plan.

Examples of Duties and Responsibilities:

• Performs/Assists the patient whose needs are not medically based with personal hygiene
• Assists the patient with unskilled positioning, transfers, ambulation, and exercises
• Performs/Assist with unskilled bowel/bladder care
• Assist with nourishments including meal preparation
• Performs household tasks
